The role
Autonomous, high-impact clinical care in participant homes
Provide direct clinical nursing care to NDIS participants with complex health requirements across Melbourne's western suburbs, including enteral nutrition, tracheostomy care, and complex wound management.
You will collaborate closely with Allied Health clinicians, GPs, and participant families to build individualized clinical care plans and train support workers on high intensity skills.
Key clinical responsibilities
Complex enteral nutrition management including PEG/PEJ feeding, tube maintenance and stoma site care.
Tracheostomy management, airway suctioning, and respiratory support monitoring.
Complex bowel care, urinary catheter management, and subcutaneous medication administration.
Clinical assessments, comprehensive care plan reviews, and high-intensity competency sign-offs for support workers.
Multidisciplinary case conferences with hospital discharge teams, allied health therapists, and families.
What you will bring
Current AHPRA registration as a Registered Nurse with no restrictions.
Minimum 2 years of post-graduate clinical nursing experience in community, acute, or rehabilitation settings.
Demonstrated competency in complex bowel care, enteral feeding, or respiratory support.
NDIS Worker Screening clearance, Working with Children Check, and clean driving record.
High-level written and verbal clinical communication and stakeholder coordination skills.
Helpful, not essential
Post-graduate qualification in community nursing, rehabilitation, or palliative care.
Experience delivering training to non-clinical support workers on high intensity support descriptors.
A second language relevant to Western Melbourne communities (Vietnamese, Arabic, Punjabi).
Working one-on-one in the participant's home gives you the clinical autonomy to genuinely transform someone's daily comfort and long-term independence.
