After you submit

What happens next, and when

Referrers get named contact details in the first call, so you are never chasing a general inbox.

  1. Same business day

    A care manager reads it

    Not a coordinator inbox. The manager who would run the placement is the one who assesses it.

  2. Within 1 business day

    You get a call back

    A yes, a no, or a waitlist position with a realistic date. We do not leave referrals open-ended.

  3. Days 2 to 5

    Meet and assess

    We visit the participant, review the plan and write a roster of care with the hours costed.

  4. Week 2 onward

    Service agreement

    Signed once everyone agrees the roster. Support usually starts within four weeks of referral.

Who can refer

Anyone can start a referral. You do not need to be the participant, and you do not need the plan in front of you.

Support coordinators

Send the plan extract and we will confirm capacity against the roster before you commit the participant.

Hospitals & allied health

Discharge planning, transition care and post-acute nursing. We attend case conferences on request.

Family & carers

You do not need to understand the plan to refer. Give us the situation and we will work out the funding.

Self-referral

Referring yourself is the most common route. Nothing is committed until you have met the care manager.
