The role
Own a portfolio of supported living homes with true person-centered care
Manage the operational and care delivery across a dedicated portfolio of Supported Independent Living homes in Melbourne's west.
Lead House Supervisors, ensure compliant Rosters of Care, coordinate plan reviews, and serve as the trusted primary contact for participants and families.
Key leadership duties
Oversee participant care plans, health management protocols, and roster of care compliance.
Supervise and support house leaders, conducting regular house audits and quality checks.
Facilitate participant onboarding, move-ins, and compatibility assessments for shared living.
Liaise directly with Support Coordinators, allied health clinicians, and family representatives.
Manage incident reviews, safeguarding measures, and continuous quality improvement initiatives.
What you will bring
Bachelor or Diploma in Disability, Social Work, Nursing, or related discipline.
3+ years experience within NDIS Supported Independent Living (SIL) operations.
In-depth understanding of NDIS Practice Standards, Restrictive Practices, and SIL funding models.
Exceptional stakeholder management, crisis handling, and problem-solving skills.
NDIS Worker Screening clearance and Victorian driver licence.
Helpful, not essential
Proven track record in SIL vacancy matching and transition planning.
Experience with positive behaviour support and complex health management frameworks.
